Cancellation
In order to provide you quality care, your time as well as ours is extremely valuable. Our commitment to adhere to the specific scheduled time is of the utmost importance to us in providing you the quality care you desire.

Please be cordial and courteous and notify us at least 24 hours in advance, if you cannot attend your scheduled appointment.
If you do not show up to the scheduled appointment, you will be subject to a NO SHOW fee.

In Network
We are currently in network with Aetna, Blue Cross Blue Shield, Cigna, and most other health insurances. Services may be covered in full or in part by your health insurance provider or employee benefit plan.

Out of Network
Insurance can be confusing, so here's a quick reference for you.

For out of network clients (meaning, if you have a provider that is not in network with Discovery Counseling Services LLC) we offer direct billing to your provider, given your payment in full (out of pocket fee) for each session.
You may be eligible for a partial reimbursement for your session(s) directly from your provider.
We recommend you contact your provider directly to clarify your benefits prior to meeting with one of our clinical staff. If you are attempting to utilize your insurance benefits; in network or out of network.

Each of these forms need to be complete or acknowledged prior to seeing a Clinical Staff Member.

All forms are provided at the office location. However, if you would like to print out a copy to fill out prior to your / family member's appointment or in case you would like to make an update to a current form. Please open the link below; print out, fill out, then provide the copy to Discovery Counseling Services LLC so the file is up to date.
